REBATE SUBMISSION INSTRUCTIONS

Steps to qualify for the rebate are easy!

1. Complete the REBATE FORM
2. Provide Proof of Purchase* from Distributor where the subscription card was purchased.
3. Provide Proof of Purchase* from APP STORE
4. Provide the PHYSICAL subscription card purchased from Distributor
5. Mail items 1 through 4 within 60 Days of original purchase from Distributor and APP Store (which ever is later)
6. Receive a check in the mail to the address provided on the REBATE FORM.

Proof Of Purchase can be physical copies of receipt from Distributor and App Store. As an alternative, ‘Screen Shot’ images can be emailed to Rebates@BidMyVid.com; all physical subscription cards and Rebate Forms must be mailed. Rebates are for DISTRIBUTION LOCATIONS that are currently participating and compliant to DISTRIBUTION AGREEMENT with BidMyVid®. Rebate Terms and Conditions are subject to change without notice.
